HUNTINGTONS VICTORIA

Huntington’s Victoria (HV) is a not-for-profit organisation which supports and assists people affected by Huntington’s disease (HD). Through the model of care which has been developed in collaboration with the Neuropsychiatry Unit, HV can ensure that their clients receive the right care at the right time. The purpose of the collaborative model of care is to provide best practice care for individuals with HD at all stages of the disease process.

The core components of the HV/RMH model are:
• Promoting optimal physical and mental health
• Capacity building and opportunities to:
o Improve social participation
o Maintain or promote a person’s capacity to live as independently as possible
• A holistic, person-centered, approach to care
• Promoting active participation in goal setting and decision making
• Timely and flexible services that respond to a person and their families needs in order to maximise independence and support carers
• A collaborative response to need in order to benefit clients utilising the service

The HV/RMH collaboration allows for both services to work within their existing structures in order to meet the needs of the HD community. Reviews of the collaboration have shown that it provides opportunities for care coordination as well as increased input for complex clients (e.g. by providing case review meetings on a regular basis). This has resulted in a more responsive, accessible and equitable service to the HD population.
